Clinic Sterilisation Room Setup: Dirty-to-Clean Workflow

An autoclave only sterilises what has been properly cleaned and packed. Here is how to lay out a clinic reprocessing area that works.

One-way flow: dirty to clean

Instruments should move in one direction — from the dirty zone, through cleaning and packing, to sterilising and clean storage — without crossing back.

1. Receiving and cleaning

A sink for soaking and manual cleaning, and ideally an ultrasonic cleaner or washer-disinfector. Wear gloves, apron and eye protection.

2. Inspection and packing

A clean, dry bench with a magnifier and good light. Check for residue and damage, lubricate handpieces, then pouch and label.

3. Sterilising

The autoclave, with space to load and unload, ventilation clearance and a nearby water supply for the reservoir.

4. Clean storage

Closed cupboards or drawers, off the floor and away from sinks, for dry pouched instruments.

Equipment checklist

  • Deep sink with a separate hand-wash basin.
  • Ultrasonic cleaner or washer-disinfector.
  • Instrument brushes, detergent and personal protective equipment.
  • Packing bench, pouches and a heat sealer if you use heat-seal pouches.
  • Autoclave of the right class and size, with a water supply system.
  • Indicator strips, Bowie-Dick or helix test and spore tests.
  • Logbook or electronic record of every cycle and test.

Frequently asked questions

Do I need an ultrasonic cleaner before the autoclave?
It is strongly recommended. Steam cannot sterilise through blood, tissue or debris, and ultrasonic cleaning reaches hinges and serrations that brushing misses.
Can the autoclave sit next to the sink?
Keep it on the clean side, away from splash. The flow should go from dirty sink to clean bench to autoclave to storage.
How big should a clinic sterilisation area be?
It depends on volume, but it must allow separate dirty and clean zones. Even a small clinic benefits from a dedicated corner rather than a shared bench.
